The shareholder or proxy should head to one of the Specialized Branches that deal with securities or, if there isn’t one of these in your region, to any Itaú branch. The originals or notarized copies of the following documents of both the donor and recipient should be brought along:
•Valid photo ID;
•CPF;
•Proof of residence, with ZIP code included and no more than 60 days old. In case the proof of residence is in the name of a spouse, a marriage certificate must be provided;
•Investor Identification Form: each party is required to fill out one form each, which should be signed before a notary public*
Additional documents:
•Marriage certificate of donor (in the form of a notarized copy);
•Proof of recipient’s bank information, that is, a document that provides proof of bank branch and bank account numbers (e.g., bank card, cancelled check, or bank statement).
Supporting donation documents:
•Original or notarized copy of the Donation Agreement;
•Donation Form: should be delivered and signed by both parties (donor and recipient) and notarized*. If the donor is married under the community property regime, the spouse should also sign. A separate form is needed for each recipient;
•Original or notarized copy of de Deed of Donation;
•ITCMD tax collection form (notarized copy).
If the request is carried out by a proxy, the person holding power of attorney will also need to bring the originals or notarized copies of his/her:
•CPF;
•Valid phot ID;
•Original Power of Attorney document specifically for this transaction.
Itaú Unibanco may request documents in addition to those cited above when deemed necessary.
Upon the completion of the transfer, a statement will be sent to the registered address.
*The person should sign the document in the presence of the notary
